Scheduling Information

Dr. Nakisbendi is licensed in the following states: Washington, Colorado, Idaho, Arizona, Tennessee, California, and Pennsylvania. If you have not seen Dr. Nakisbendi in the last year, you must physically be in one of these states at your initial visit to receive care via telemedicine.​

Becoming a New Patient

All new patient appointments are scheduled on Wednesday and Friday mornings. After selecting a time, your request will be reviewed and approved by our staff. You will receive confirmation of approval or denial via email and the CHARM patient portal.

If you are interested in scheduling a Sottopelle consultation, please complete the contact form below and a member of our team will follow up with you.

Existing Patients

If you are an existing patient, please schedule a telehealth or in-person appointment through your CHARM patient portal or link below.

Appointments may be rescheduled up to 48 business hours in advance. Any cancellations inside this window will result in a full appointment charge.

Appointment reminders are sent via email 3 days in advance.

Insurance and Payment

Dr. Nakisbendi does not participate with insurance plans, including Medicaid or Medicare. NakisbendiMD operates as a fee-for-service practice.

Dr. Nakisbendi can order basic laboratory testing and imaging through private insurance, Medicaid, and/or Medicare. After each appointment, you will receive a superbill, which you may submit to your insurance provider for potential out-of-network reimbursement.

New Patient Forms & Medical Records

We understand that completing the new patient questionnaire and consent forms can be time-consuming. However, submitting these documents ahead of your appointment allows Dr. Nakisbendi to review your information in advance and make the most of your time together.

Please note that all required forms must be completed no later than 7 days before your scheduled appointment. If they are not completed by this time, your appointment will be rescheduled.

Once your appointment is scheduled, please gather any relevant medical records and follow the instructions provided to upload them to your CHARM patient portal. Our office may contact you to reschedule your appointment if questionnaires, consent forms, or required information are incomplete or not submitted thoroughly.
